INFORMATION NECESSARY FOR MATCHING GRANT REQUEST

1. Name of Kiwanis Club submitting grant request

2. Description of program needing grant funding

3. Kiwanis fund raising project, amount of funds raised, and number of Kiwanians involved.

4. Impact on the local community if project is approved and conducted.

5. A bank check from the Kiwanis Club to the Alabama Kiwanis Foundation for the amount which is to be matched .( i.e., Kiwanis Club submits check for $1000, Foundation considers and approves sending the club a $2000 check). Maximum amount Foundation can match is $1000.
